About THE REACHOUT PROJECT
The Reachout Project is a non-profit organization that depends on grants and donations to enhance the prevention of Chronic Illness in our communities so that we may detect it early enough to slow progression through advanced prevention care or prevent it from ever happening.
These include Comprehensive Diabetes Care, Medication Management, Chronic Care Management, and other ancillary support services.

- This also helps to keep the patient within the network as they continue healthy living changes that are measurable and monitored digitally.
